CARILLON MIAMI WELLNESS RESORT ATTESTATION

We are taking the prevention of spread of COVID-19 (the “Virus”) seriously in our community and workplace and we are restricting access to Carillon Miami Wellness Resort from anyone with knowledge that they have a COVID-19 infection or have recently been exposed to the Virus. By entering the premises, you and anyone in your traveling party are affirming and attesting to the following:

• Neither you nor anyone in your traveling party have been exposed to the Virus or any individuals known to have been exposed to the Virus.
• Neither you nor anyone in your traveling party are currently experience or displaying any of the following symptoms now or in the last 14 days: cough, fever, shortness of breath or difficulty breathing or other less common symptoms including muscle pain and gastrointestinal symptoms, like nausea, vomiting or diarrhea.
• You and anyone in your traveling party will immediately notify Carillon Miami Wellness in the event you or anyone in your traveling party experience any of the symptoms listed above or begin to feel ill.
• Neither you nor anyone in your traveling party have, in the last 14 days, traveled to (i) areas of substantial community spread requiring quarantine per the Florida Department of Health or (ii) traveled internationally – based on the current U.S. State of Department Level 4 travel advisory for all international travel due to the global impact of COVID-19.
• Updated on March 2022 as per the latest CDC recommendations: Masks are optional. Wear a mask based on your personal preference, informed by your personal level of risk. 

If you or anyone you are traveling with cannot affirm and attest to all of the above statements, we will gladly reschedule or cancel your reservation so that you may stay with us once resolved.
